This policy is meant to inform you of your rights around the collection, use and disclosure of your personal information, under the federal and municipal protection of personal privacy acts, including the Freedom of Information and Protection of Privacy Act and Personal Health Information Protection Act.
This information applies to anyone who has accessed this website or is a current or past client of Soar Above Counselling. The information contained on www.soarabovecounselling.ca is intended for adults 18 years of age and up. Soar Above Counselling does not provide counselling to individuals under the age of 18.
PERSONAL INFORMATION – USE AND COLLECTION
Soar Above Counselling will collect and use information only when it is relevant to the provision of counselling services and consultation services as requested by you. Your personal information may be used by Soar Above Counselling to provide counselling services or information to you, to respond to emails or provide resources to augment your therapy experience, to arrange and conduct consultations, to manage the waiting and cancellation lists and to contact you when your name comes up to start counselling.
Examples of the information collected includes, but is not limited to, your name, address, date of birth and email address. If you have any questions regarding the information requested by Soar Above Counselling, please email at firstname.lastname@example.org.
Soar Above Counselling understands the importance of ensuring your personal information is protected. We have taken active steps to protect this information. As per the requirements of her professional college, Gabrielle Fecteau ensures that all client/clinical information is stored using an online practice management system which has secure servers in Canada.
INTAKE AND COUNSELLING
Soar Above Counselling has partnered with OnCall Health to ensure that client information gathered in the context of online counselling sessions is properly encrypted and stored.
Information in the intake forms are gathered to ensure that you are the person who is seeking counselling and that you are a residence of Ontario where the social work registration of Gabrielle Fecteau (owner and counsellor) is valid. This information is also requested to ensure that you are contactable outside of the session if needed or during an emergency.
As we are an online counselling service, we use email to communicate with our clients between sessions. Nevertheless, email is not considered a secure and confidential way of communicating. For the protection of your privacy, please do not email information you would not be comfortable discussing in a public space. Gabrielle Fecteau is happy to confidentially discuss your concerns either in a session, or if they can be addressed in a quick conversation over the phone or OnCall services.
You may choose to share any information you deem important about your personal situation, past or present, for the purpose of providing counselling and collaborating with you on a treatment plan. This information remains confidential unless otherwise indicated by law (see section below, “Confidentiality”).
DISCLOSURE OF PERSONAL INFORMATION
DISCLOSURE TO THIRD PARTIES
Soar Above Counselling is not authorized to disclose your personal information to any third party without your express written and signed consent. Soar Above Counselling cannot, and will not, respond to request from concerned family members or friends about clients’ attendance, disclosure in session or established treatment plans and progress. This also applies to family members or friends who are paying for the client’s sessions.
There are restrictions, albeit rare, to confidentiality which involved disclosure to third parties; these are detailed in the section below, “Confidentiality.”
Your identity, attendance and all personal information communicated by you in counselling is strictly confidential, with the following exceptions:
- Expression of the desire to harm yourself or someone else.
- Suspected or know of child or elder abuse/neglect. The reporting of such suspicions is required by law.
- Subpoenaed counselling records by the court.
- Request for a clinical consultation or supervision; no identifying information about you is released.
It is possible that your insurance company contact Gabrielle to request confirmation of sessions that have been claimed via employment/extended health benefits or plans. This is to stop insurance fraud by ensuring that submitted sessions have occurred. If you do not want us to verify sessions with your insurer, please let us know but note that this may result in you being denied reimbursement for sessions that you have claimed.
Should Gabrielle be required to retain the services of a collection agency, seek legal counsel or to retain the services of a bookkeeper who handles copies of receipt, information may be provided to third parties.
